What is Accutane?
Accutane is a form of Vitamin A in the form of a capsule. Its generic name is Isotretinoin, and it belongs to a class of drugs known as retinoids. It is used to treat cystic acne, also sometimes called nodular acne, which is when excess production of sebum leads to chronic acne on the skin.
Accutane works by lowering the sebum production to keep the skin dry, but it can also cause severe dryness to other parts of the body as a side effect. It is also extremely harmful to foetuses and could even cause death so in order to take it, a negative pregnancy test is necessary first.
How to Take Accutane
Isotretinoin is taken orally in the form of capsules, twice a day with a glass of water. Only take it as prescribed by the doctor. Never take it outside of your prescription or for longer or shorter than the prescribed duration. Your prescription must be filled within 7 days of the doctor writing it, and you can only receive a 30-days worth of doses at a time.
The approximate duration of therapy is supposed to last 20 weeks. Adults can take between 0.25mg to 0.5mg per kg of body weight twice per day. The maximum dose for adults is 2mg per kg a day. Teens (12 or older) can take the same dosage of this medication.
Accutane Side Effects
To the patient using Isotretinoin, the acne might worsen at first but it will improve visibly soon after. It can also cause dryness on other parts of the body like chapped lips or dry nostrils which may lead to nose bleeds. The patient could find they are sensitive to sunlight and have thinning hair.
The more dangerous form of side effect is the birth defects that can happen if the patient is pregnant during or anytime within a month after using Isotretinoin. The patient must use two highly effective methods of contraception during the period mentioned above.
Birth defects might include abnormalities of the skull, ears, eyes, face, CNS, cardiovascular system, and thymus and parathyroid glands. IQs of less than 85 have also been reported in children whose mothers used Accutane while pregnant.
Isotretinoin also causes internal problems in the user. Added pressure to the skull leads to severe headaches, mood swings, pressure on the brain and other mental health issues. The rest of the body can also suffer, by facing musculoskeletal pains, stomach aches, breathing problems, build up in the blood and other internal damage.
However, it’s best to keep in mind that these side effects are very rare and affect only a handful of people. Consult your doctor before you administer the drug to avoid any of these side effects.
Contraindications of Accutane
If you are already taking other forms of Vitamin A, taking Accutane might cause bone damage. This might include anti-seizure drugs like phenytoin. Using contraceptives during your usage of this drug might be a good idea. However, Progestin-only pills might not be enough.
Taking St John’s Wort might also make contraceptives less effective. Steroids, tetracycline antibiotics and vitamin supplements should also be avoided.
Before Taking Accutane
Make sure you are not pregnant and are not going to be during or a month after stopping Accutane. 2 negative pregnancy tests are required for your prescription to be approved. After that, each month of using the drug, pregnancy tests are done to ensure your safety.
Being on Isotretinoin also requires you to take biweekly liver function tests as liver enzymes can be elevated during the use of this drug. Irregularities in blood sugar levels and CPK have also been observed during this period but the causal relationship is not confirmed.
Let your doctor know if you have a history of depression or mental illness, asthma, liver disease, diabetes, heart disease or high cholesterol osteoporosis or low bone mineral density, an eating disorder such as anorexia, a food or drug allergy, or an intestinal disorder such as inflammatory bowel disease or ulcerative colitis.
Do not breastfeed while on this medication. It is also not approved for anyone under the age of 12.
This drug may contain allergens, especially if you have a vitamin A allergy. Isotretinoin also contains soybean derivatives and parabens. If you are allergic to peanuts you might also be allergic to soybean.
Do not donate blood during and a month after stopping Isotretinoin. It builds up in your blood and might be fatal to the receiver or their unborn child if they are pregnant.
Accutane makes you more sensitive to everything, so you may feel discomfort in doing regular things like wearing contact lenses or going out in the sun without protection. It is also best to avoid skin smoothening techniques during this time such as microdermabrasion and laser as the procedures might have more harmful effects than usual.
In Case of Missed Dose
If you miss a dose of Accutane, simply take it unless the timing is too close to the next one. Do not take more than one dose at a time.
Just a single dose of Accutane can be extremely fatal for an unborn baby. If you suspect you might be pregnant, stop taking the medication right away and call your doctor.
Only patients who are capable of understanding and following the rules of precaution are permitted to use Accutane, but not for longer than 20 months. A form must be filled-up by the patient as proof as well as taking multiple lab tests.
In Case of an Overdose
If you or someone you know has been poisoned by Isotretinoin then you might be experiencing vomiting, stomach pain, facial flushing, headache and loss of balance. If the person is having breathing problems, call emergency medical help.
Store this medicine in a dry place in room temperature. Do not freeze it or refrigerate it and do not keep it in a bathroom. Keep out of reach of children under 12 years old. Do not share this medication with anyone else, even if they have the same symptoms as you. You will only get exactly as much as you are prescribed so sharing pills is very risky.
